What is the project about?
Western HealthLinks is a pilot program funded by the Victorian Department of Health and Human Services (DHHS) to improve health outcomes for people living with chronic disease. It is a three-year pilot and aims to reduce unplanned hospital readmissions.
Who’s involved?
Western Health (WH) initially partnered with Silver Chain Group (SCG), a non-profit organisation with a proven track record in delivering in-home health and care services across Australia, to provide this innovative model. Western Health now operates without the Silverchain Group on this program.
What are the outcomes?
Western Health led support for a different approach through the Victorian Government’s HealthLinks program, to improve our capacity to provide care outside of the inpatient hospital setting.
Commencing in November 2016, more than 2,000 patients have been enrolled for the Western HealthLinks program and more than 500 have received active support following an admission to hospital.
