What is the project about?

The Western Health catchment has high rates of chronic disease including chronic kidney disease, diabetes and cardiovascular disease. We are running an implementation program of Future Health Today a clinical decision support system and quality improvement software for kidney disease, diabetes and cardiovascular disease in primary care. The program is supported by education and specialist outreach support. Through this project we aim to partner with primary care to improve the testing, diagnosis and management of kidney disease, diabetes and heart disease.

Who’s involved?

Future Health Today is a collaborative project between the Department of General Practice at University of Melbourne and Western Health. For this implementation program we are partnering with 20 general practices in the Western Health catchment. Western Health clinicians will also be providing an education program and outreach support.

What are the anticipated outcomes of the project?

We are aiming to improve the detection, diagnosis and management of kidney disease, diabetes and heart disease in participating practice sites. We will also be linking primary care and tertiary care data to understand the health service impacts of investing in earlier detection and management in primary care.
